On Oct. 14, the Centers for Medicare and Medicaid Services announced more than $1 million in Rural Health Transformation Program funding for physician recruitment and retention in rural Delaware. It follows the Nebraska and Missouri awards announced a day earlier.

For residents and medical students weighing where to practice, Delaware's piece of the program is worth a close read, mainly for what the release leaves out.

What Delaware announced

The release describes two educational awards for eligible medical students in the program's first year, and funding for an expected 61 housing and relocation awards for eligible applicants in graduate medical education programs. Recipients commit to five years of practice in rural Delaware, with at least 50 percent of their practice serving qualified rural areas.

The money comes from the $50 billion rural health fund created under the law CMS calls the Working Families Tax Cuts Act, and it is one part of Delaware's larger fiscal 2026 award. CMS lists the state's priorities as workforce recruitment and training, rural facilities and access, new care and payment models, community-based and preventive care, and rural health technology. Health and Human Services Secretary Robert F. Kennedy Jr. said in the release, “We are giving states the flexibility to build solutions around the people they serve.”

What the release does not say

The release gives no individual award amounts, eligibility criteria or application path. States set those terms, so the details will come from Delaware, not CMS.

A five-year service commitment is a contract term. Read it like one before accepting housing or relocation money.

What to check before you sign

Any trainee considering a service-linked award should ask what happens if the commitment is not met, whether repayment is required and how the 50 percent rural-practice requirement is measured. Also ask whether the commitment can be met across employers, since a job change inside a five-year window is common. Our guide to reading your first employment contract covers the clauses that tend to matter.

Watch the Delaware state agency for the application and eligibility terms. Until they are published, treat the figures as announced intent, not an offer.
